Description of Service

FormAgents is an AI-powered form builder platform that allows you to create forms through natural language conversations. Forms can be assigned phone numbers, enabling respondents to submit information via voice calls 24/7.

Account Registration

To use certain features of FormAgents, you must create an account. You are responsible for maintaining the confidentiality of your account credentials and for all activities that occur under your account. You agree to provide accurate, current, and complete information during registration and to update this information as needed.

Acceptable Use

You agree not to use FormAgents for any unlawful or prohibited purposes, including but not limited to:

• Sending spam or unsolicited communications
• Collecting or distributing illegal content
• Distributing malware or malicious code
• Collecting sensitive personal data (such as Social Security numbers, financial account information, or health information) without proper authorization and security measures
• Impersonating others or misrepresenting your affiliation
• Interfering with or disrupting the service

AI-Generated Content

FormAgents uses artificial intelligence to assist with form creation. While we strive for accuracy, AI-generated content may contain errors or require adjustments. You are responsible for reviewing and approving all content before publishing your forms. FormAgents is not liable for any issues arising from AI-generated content that you choose to use.

Intellectual Property

You retain ownership of the forms you create and the data you collect through FormAgents. FormAgents retains ownership of the platform, including all software, designs, trademarks, and other intellectual property. You grant FormAgents a limited license to host and display your content solely to provide the service.

Payment Terms

Paid features are billed on a monthly or annual basis as selected during subscription. Payments are processed through Stripe. All fees are non-refundable except where required by law. You may cancel your subscription at any time, and access will continue until the end of your current billing period.

Service Availability

We strive to maintain high availability, but we do not guarantee uninterrupted access to FormAgents. The service may be temporarily unavailable due to maintenance, updates, or circumstances beyond our control. We will make reasonable efforts to provide advance notice of planned maintenance.

Limitation of Liability

To the maximum extent permitted by law, FormAgents and its officers, directors, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of the service. Our total liability shall not exceed the amount you paid to FormAgents in the twelve months preceding the claim. The service is provided "as is" without warranties of any kind.

Termination

FormAgents may suspend or terminate your account if you violate these terms or engage in conduct that we determine is harmful to the service or other users. You may cancel your account at any time through your account settings. Upon termination, your right to use the service will cease, and we may delete your data after a reasonable retention period.

Dispute Resolution

These Terms of Service are governed by and construed in accordance with the laws of the State of Florida, without regard to its conflict of laws principles. Any disputes arising from these terms or your use of FormAgents shall be brought exclusively in the state or federal courts located in Nassau County, Florida, and you consent to the personal jurisdiction and venue of such courts. You agree to attempt to resolve any disputes informally before pursuing formal legal action.

Changes to Terms

We may update these Terms of Service from time to time. We will notify you of material changes by posting the updated terms on our website and updating the "Last updated" date. Your continued use of FormAgents after changes are posted constitutes acceptance of the updated terms.
